PLEASE FOLLOW THESE STEPS FOR PROPER REGISTRATION:
1. CHOOSE A PACKAGE: Read the options for the
different packages offered and choose the option that best fits your interest. Then, check the designated box on the registration form.
2. REGISTRATION FORM: Highlight and print the selected registration form. Completely fill out all necessary blanks on
the registration form. Not all may apply to you.
3. TERMS & CONDITIONS CONTRACT: Highlight and print the selected terms and conditions document. Completely
read the Terms & Conditions Contract page and sign and date all necessary signature blanks. All participants must sign their own contract.
4. MINOR'S RELEASE FORM: All Minor's
must completely fill out the minor's release form. BOTH legal parents must sign the document, before departure.

5. TRAVELER’S PROTECTION WARRANTY: Thoroughly read the Traveler’s Protection Warranty, and then voluntarily accept the terms by signing
at the bottom of the “Terms & Conditions” contract. (Optional)
6. SEND DOCUMENTS:
Send all payments,
registration forms, and Terms & Conditions contract to: All

*Each traveling passenger must use his/her own registration form and
sign his/her contract page.
**Please register by March 12th 2005 if you are interested in attending World Youth Day
2005. Late registrations are accepted under certain conditions.
